The first part<\/strong>: The “Personal data decreeing person” and “Medical data decreeing person” come directly from the database and cannot be changed here<\/strong>, but only in your member area under safeblood.us\/en\/profile<\/a> (or with your country code) where you press “edit”.<\/p>\n

The second part<\/strong>: In the text field, the actual text<\/strong> of your living will is given as a sample. You can change whatever you like<\/strong>. In any case, you must complete<\/strong> the points where there is a “…..<\/strong>“. Behind the “…..” is explained in each case what you have to put there instead of the points (e.g. “first name + surname”<\/em>). You then delete this oblique explanation text.
\nIf you don’t have or don’t want a “trusted person”, delete everything related to it.
\nIf your trusted person does not have the same blood group as you, you must change this passage here in the text or delete the entire passage.<\/strong>
\nVery important<\/strong>: When you have customized your living will, save it<\/strong> by pressing the red “Save changes” button<\/strong> – if you do not do this, everything will revert to the original text.
\nAfter<\/strong> you have saved your changes, you can upload a scanned signature<\/strong>. To do this, select the file that contains your signature (Choose file) and then click on \u201cUpload signature\u201d. After you see the message \u201cSignature successfully uploaded\u201d, your living will is up-to-date.
\nIf you do not yet have a scanned signature, make one on white paper and photograph or scan it before you can upload it.
\n(Please do not upload the signature under \u201cUpload ID\u201d. If you see this option, you are on the wrong page).<\/p>\n

We recommend that you print out the living will<\/strong> at the end and sign it by hand so that you can deliver the printed version later if this is required.<\/p>\n
